Susannah Azofeifa PT, DPT
ProActive Rehabilitation and Wellness
My name is Susannah Azofeifa. I graduated from Brenau University in Gainesville, GA with my Doctorate of Physical Therapy. I have been practicing in outpatient orthopedics and treating the women's pelvic health population since August of 2021. I treat general orthopedic conditions such as joint pain, post-surgical, and balance/strengthening. With women's pelvic health, I treat a variety of conditions. This includes, but is not limited to, pregnancy and postpartum, urinary incontinence, urinary urgency and frequency, pelvic pain, and pelvic organ prolapse. I plan to get my Pelvic Rehabilitation Practitioner Certification (PRPC) after 5 years of practicing.
